The list of failed/terror-supporting countries is specified by the US State Department. The list Trump had was compiled by the Obama Administration.

Pipes' opinion that it should be applied to ALL those coming to the US is too difficult. Ever stand at a TSA line at the airport? Go get my drift.

Because terrorist come from mostly Middle East countries, it makes perfect sense to extreme vet from those countries.
